Including extra fields to a 1password Login

I was just logging in to the American Airlines site and noticed that they require 3 fields; username, last name, and password. Is there a way to add another login filed in 1password? This is what it looks like.

    Hey ajahn ,

You can add custom fields to a login item in 1Password. Which field is not being autofilled for you? If it's the last name field, try this:

    1. Open your 1Password app.
    2. Locate the login entry for this website, select it and click on "Edit".
    3. Under the username, password and website URL, select an empty "Label" and name it "Last name" (case sensitive, that's how it's written in the website).
    4. Input whatever you want it to fill in the empty field that belongs to that label.
    5. Click on "Save" to save the changes, then test autofilling on the page and see if it works.

In case it didn't work, let me know the following:
    - Which field is not being autofilled.
    - What browser are you using.
    - What is the version of the 1Password extension in that browser.
