1Password not saving updated password - identified a reason = 3D Secure
I had my Tesco password stored in 1Password. When I updated the password, 1Password didn't offer to update the password. As part of my troubleshooting, I deleted the login and everything to do with Tesco in 1Password. I then tried to login and see if 1Password would save it - which it didn't.
I then wondered if I had somehow told 1Password not to save logins for this web site - and indeed I found in the Automatically ask to save new Logins was checked but had Tesco.com listed in the exclusions.
Then I realised what had happened.
3D Secure.
When going through the checkout of various web sites, 3D Secure is required to confirm credit card authorisation. Annoyingly, having entered the 3D Secure answers (which is usually 3 characters from the password, e.g. 1st, 4th and 4th character, ie. nothing worthy of saving within 1Password) 1Password asks to save it. This was so inconvenient that I started telling it 'Never offer to save for this site' thinking it would realise I was referring to the 3D Secure page. But as I've discovered, it's treating it as though I don't want it to save any login information for Tesco.com - or any other shopping web site that uses 3D secure.
I'm not sure how 1Password can fix this - but it's a huge problem. Without blocking 3D secure there is this constant annoying window asking to save the useless information entered in 3D secure, but by blocking the site it leads to problems with passwords not being updated.
I hope this will shed some light on what is perhaps a commonly encountered problem that hasn't get been identified.

Just to complete this information - when going through 3D Secure and 1Password offers to save the login information, it tries to name it as Arcot. This gives the impression that it recognises it's a different site (I'm well aware that the URL is for the site that the purchase is being made from). But by telling it never to save login information for this 'Arcot' site, actually blocks 1Password from saving any future login information for the site being purchased from - ie. won't save future password updates.
