Just put the item into Edit mode and you will see a blank template for a new section at the end of the item. As soon as you start completing it another blank one is added below. Same goes for any fields that you add.

I'm not sure I understand the question either. But there is one thing that may help. When you have added text to a custom field and moved on to the next one you will see a drop-down arrow to the right of the text field which will allow you to change the field from text to password. If you change it to password you can obscure and reveal the contents in the same way as a password field.

I think @samb is asking for new fields whose types are menu-pick, where a preset list of choices is available in a menu, rather than the standard string value field (think of the Credit Card's Type field with choices of Visa, MasterCard, etc.
Sorry @samb, this doesn't exist as an option in 1Password yet.

Unfortunately, custom fields were introduced in 1Password 4, so you will not be able to create fields in 1Password 3. If you need to store any sort of unique information within an item in 1Password 3, I suggest using the Notes field.
