Are all future upgrades free?

No. Previous practice has been that minor version upgrades (e.g., 3.3, 3.4, 3.5) are free but major version upgrades (e.g., 3 to 4) are not (but are usually discounted for existing paid users). That said, the major version upgrade from 4 to 5 was free.
In my experience as a user (I'm not employed by AgileBits) the company has been extremely fair in paid upgrades. There are many updates and improvements made to the program regularly and frequently and paid upgrades do not occur that often.

Like Stephen said above, most updates are free but we do occasionally have (discounted) paid upgrades for major versions, to support continued development of 1Password. But you're able to continue using the version you originally paid for as long as you want.
