Have you tried re-creating that item on your Mac and then deleting the old item to see if the new one will then sync?

I'm sorry to hear that you've hit this issue. Is there any chance that this item was deleted on the iPad at one point? We found a bug in 1Password for Mac 5.0 and 1Password for iOS 5.0/5.1 where in some cases, if an item was deleted on one device, but another device revived it (we play it safe in some cases where we aren't sure you actually meant to delete it), then that revived item doesn't get pushed back to the device that deleted it. This issue was fixed in Mac/5.1 and iOS/5.2 which should be released soonish.
The workaround would be to re-create the item, and that should get pushed.
If there's no chance that this was ever deleted, then we'll need to do some more digging to see what's going on here. We fixed many issues in 5.1/5.2 though, so it's possible it's one of the other many issues we fixed up that's causing it. And if it isn't, I'd love to get it on our list of things to fix up.

Don't think it was the iPad thing. But just saw a button at the bottom of that item: Convert to Login. When I clicked that it immediately synced. It was listed as a Login so not sure how this happened. But problem solved. Thanks guys.
