equifax 90 day alert

As a security precaution, I fill out the Equifax Request an Initial 90 Day Fraud Alert (alerts.equifax.com) every 90 days.

Using my 1Password identity does not fill out all the fields on the form. These fields include Initial, Social Security Number, and three Contact Telephone Numbers.

Are there any techniques to map fields in my identity to fields in the equifax form.

    I'm sorry you're having some trouble filling certain fields from an Identity item! I tried opening alerts.equifax.com but received an error saying that site isn't available. Can you please let us know the full, exact URL for the site? Thanks!
