master password reverted to a former one

My master password recently reverted to one I'd been using 6+ months ago. I only found this out by entering every master password I'd ever used in the hope that one of them would work. I have no idea why it happened (I hadn't recently restored anything from backup). Is this a thing that's happened to anyone else? Is there anything I can do to prevent it from happening again?

    Please tell us:

    1. what version of 1P you are using on your Mac (go to 1P > About 1Password and tell us exactly what it says there);
    2. what version of 1P for iOS you are using (go to the 1P Settings screen and look towards the bottom for the version number);
    3. your OS X and iOS version numbers;
    4. how you sync your 1P data between your Mac and your iOS device(s) (Dropbox, iCloud or local wifi sync);
    5. whether anything you did on your devices (e.g., updating 1P on a device) coincided with the master password problem.

    I'm sorry that this happened to you. On top of the information that @stephen_c mentioned, I'd like to add an additional question.

    1. Do you remember what you were using to sync 1Password when you did the Master Password changes (if different than your current sync setup).
