ADP login and notes disappeared. Now unable to access W-2.
I was registering for ADP in order to access my W-2 and do my taxes. I attempted to convert the generated password to a login via 1Password mini, adding the username and appending the “answers” to their security questions as a note (e.g., “father’s name/K1-hev-meik-voict-oshk-ad-nurr).
When I went to log back into the site, this newly-created login and all of the note information with the answers to the security questions is gone. I can’t login because I don’t have the password that was generated and I’m now unable to reset to a new password because I can’t answer the security questions.
I checked the most recent backup to see if it contained this newly created login, but nothing was there either.

Have you looked in your Passwords category to see if the generated password is there? That category saves passwords not specifically linked to a saved login item. (I'm assuming you're using either 1P4 or 1P5.)

Using Version 5.1 (510035) and unfortunately, no, nothing related to ADP in the Passwords category either.

Have you checked the Trash in 1Password. I can't imagine how it could have happened but is there a Login item in there at all? I know there is a Move to Trash button in the edit pane that upon testing doesn't require a confirmation.
What I'm a little worried about are timings. Say you created the Password item and then pretty much tried to convert it straight away, then given 1Password only creates a backup at most once a day if you've restored a backup without first creating a snapshot then I can't see how it can be retrieved.
To try and explain better imagine the following timeline.
- You open 1Password and it creates a backup as it realises it hasn't done so today.
- Say a couple of hours later you first create the Password item using the Password Generator.
- Minutes later, (30, 40 etc.) you convert the Password item into a Login item and something happens. Exactly what we can't say.
- You restore the last backup which was prior to event 2. If you didn't create a new Backup before this restoration then everything that happened after event 1. is now irretrievable.
