1Password doesn't overwrite the whole vault on one device with the vault from another device. If you have different data on each one and you sync them with each other for the first time, 1Password will merge them together. For example, if you create 30 items in 1Password on your Mac, and you create another 10 items in 1Password on your iOS device, and then you sync them with each other, you'll have 40 items on each one.
After the initial sync, 1Password will just copy updated information back & forth. So if you add a new item on one device, the new item will be copied to the other device. If you make a change to an existing item on one device, that change will be copied to the other device. If you make different changes to the same item on both devices before a sync occurs, it will save any conflicting changes in 'Conflict' fields in that item so you don't lose anything.
