just getting started, can I run from desktop, without being in browser?

Installing the desktop program does not install browser extension, they're two separate pieces of the puzzle. Web browsers generally do not permit automatic installation of browser extensions, the user has to install it manually.
We do offer two ways of using 1Password, you can use 1Password X, which is a self-contained browser extension that doesn't require a desktop program and connects to your 1Password.com account. Or you can install 1Password desktop app and then install the separate browser extension from here: https://1password.com/downloads/#browsers
