Safari not completing autofill on cc website

Version 4.4.2 (MAS) stopped filling in "password" information at acccountonline.com. Only username information is populating.

I'm using Mavericks 10.9.5 and Safari 7.1.
IP 4.4.2(442010) w/ Ext. 4.2.5

    This sounds like the your Login in 1Password is no longer working for acccountonline.com. This may be cause by acccountonline.com changing the login form since you saved your Login in 1Password.

    To remediate this, please refer to this article from our Knowledgebase which will guide you through the steps of updating your acccountonline.com 1Password Login.
