Browser extension and menu bar access to 1 Password problem

I have 4 bank accounts listed in the main 1Password application but only one of these accounts (always the same one) shows up when I open 1Password in the menu bar or browser extension. Not sure how long this has been occurring, I don't use the bank account info too often.

Mac OS 10.10
1Password - Version 5.0.2 (502006)

    Hi @fiddlehead,

    Could you please open the main app, edit the items not displaying in 1Password mini, and ensure that display is set to Always:
