Filling multiple form data values that never change (US Veterans health co-payment)

Hello Support!

I submitted this question about a year ago but NOW have new information included below.

I need to pay a co-pay to the VA Health Care System
I login at this site, using 1Password successfully:
https://www.pay.gov/public/home

After a few clicks I arrive at https://www.pay.gov/public/form/entry/106/25987221/
This page is covered with eleven data entry fields which are ALWAYS the same, plus an amount of payment (which may vary).
HOW can I get 1Password to save this repeating data and auto-fill on my next visit?

How can I make 1Password fill in the fields that are always the same values?

    You just need to make an 'Identity' item in 1Paswword, and create series of named fields in that entry that match the form.

    So New Item > Identity > Scroll to bottom 'add more' > Text
    Name your new Text field 'Veteran's First Name' or 'Part 1' etc. and enter the value you want in those fields.

    Then when autofilling on the page, pick the Identity you made.

    I have been doing some testing with the form you have shared and as @ajh0912 has suggested I have been able to create an Identity item specifically for this page. You will need to add some custom fields for the Account number and full name given them the same labels as I have done below:

    Let us know how you get on and if you need some assistance recreating this on your end. Once you have created it you should be able to visit the form, click on the 1Password icon in your browser toolbar and click 'Autofill' next to the identity item.
