Greetings @NeilFaiman,
Can you try something for me please, can you manually save an entirely new Login item using the steps detailed on our support page How to save a Login manually in your browser and let me know how you get along. Does it perform any differently than your existing Login item?

OK, that fixes the problem. Furthermore, I think I understand what the problem was. The new login item has an extra "website" field, with the URL https://ledgertranscript-nh.newsmemory.com/eebrowser/optionspage/opt.check.0886/php/createaccount/createaccount.php?pSetup=monadnockledger&callFrom=paywall&popup=account&callBackHome=https%3A%2F%2Fwww.ledgertranscript.com%2F&username=neil.ledger-transcript@faiman.org. Apparently the main website <www.ledgertranscript.com> is outsourcing the actual login popup to a different domain, <ledgertranscript-nh.newsmemory.com>, and without that additional domain in the 1Password login entry, 1Password (obviously) can't match the login popup to the correct login entry. Thanks!

Hello @NeilFaiman,
That was certainly one possibility I was considering  So 1Password correctly offered your original Login item because the domain referenced in the website field matched the domain of the visible URL. Each iframe on a page though is treated separately and 1Password will only fill those that also match the Login item, a protection against both filling spurious iframes the site did add as well as the possibility of something being hijacked to add a dodgy form without the site owners realising. It just wouldn't be easy to know by glncing at the page unless you'd seen this thing more than once before. As most sites also involve me needing to create a test item as the first step I gain an advantage there as well  It hopefully won't give you any trouble now.
