Auto filling certain fields of a webpage

There is a web page that I consult every day that requires my last name, first name and two numeric identity values to let me download a file.
They are not logins, and 1Password does not seem to be aware of their existence.
Is there a way that to define in 1Password an entry so I can click to auto fill these 4 fields? Or in general to fill forms?

    Hi @fahirsch!

    Have you tried using an Identity item for this? If anything, 1Password should be able to fill at least the first and last name on the website.
