1Password failing to correctly identify username more frequently
Many websites include a users's personal name (First Name, Last Name, Full Name) in account creation forms, and increasingly 1Password has been misidentifying these as the account username when saving login items. Other sites miss collecting the username at all, possibly if the site has separate pages for username and password entry/creation. Hopefully there's something that can be done about this. Some sites that have most recently been an issue are: Box, Avery, WPEngine, Sharefile. I'll update this topic with more as I recall them or experience them.

Hi @extensioncord,
Could I trouble you for URLs please. It's entirely possible there is only one option but sometimes there can be multiple places a user can reach and I've found plenty of sites where they have distinct and very different forms depending on which you reach. We'll happily test and file issues for when the detection isn't doing a great job.
