This did work up until 4-5 months ago when they changed their website.

    Can you please let us know:

    1. What browser (and browser version) this is happening with?
    2. What is the 1Password behavior on that page? What happens when you try to fill your login?

    For sure @ag_ana,

    1) Safari 12.1.2, Firefox 68.0.2, Chrome 76.0.3809.132
    2) It fills the “access code” in to the “SIN” field, then submits the form with the other 2 fields empty.

    I have tried saving the 3 items fresh, but it doesn’t seem to work.

    Thank you for the information! I created a new item for that website and I was able to fill all three fields here, including the dropdown menu. To test this, I have created the item manually in the browser, so can you please give that method a try and see if creating the item that way helps?
