Adding Field Names to 1Password Record
I use the site https://www.aa.com/homePage.do often there are three fields on the input screen shown in the image, 
. The Advantage Number (or username) and the Password are filled in from the 1Password, but I can not get the Last Name to port from 1Password - probably because I don't have the correct field name.

Can you help me find out how to add it?

Basically, you fill all three fields in, then save the login. 1Password will thereafter remember all three during autofill.

Can you please try saving the login manually and see if that helps?
