How to create logins with three fields?

I want to create a login with 3 fields as per below. Is it possible?

    Hi @mav64! Welcome to the forum!

    It is indeed possible, but to make sure that all fields fill correctly, it might be easier to create this item manually using the desktop version of 1Password, if you can.

    I tried to save the login details manually as per instructions on the desktop version but it still only saved two of the three fields. How can I save details for 3 fields? The fields saved as per my attached screenshot above were date of birth and password

    Hi @mav64

    If manually saving the item from the desktop extension doesn't do the trick then unfortunately I'm not aware of a way to do so. I'm sorry I don't have a better answer to offer at this time.
