Scanning in a document appears as (deleted item) in related items, but is actually added...

As the title states, I open Drivers License entry, select edit, then Add New File, then scan in the DL. it appears as (deleted item) and cannot be opened. When I go to documents, in there as "Scanned Document.pdf". The DL is in a Shared Vault.

    I have same issue. Took a picture to add as a related item. The .jpg shows up properly under heading: (deleted item). Can’t be opened.
