Unable to add a new password to the vault

I'm unable to add a new password to the vault from a web browser, I get this error: "Your Secret Key has changed. Please sign in to your account to re-authenticate"
A link was provided underneath the error, I clicked on it and logged into my account and this has not helped, any idea of how to resolve this?
