All of my pws on Mac are double on IOS devices '

When I add a password in my Mac laptop it shows up twice on my iPad and iPhone. I have 102 items in my Mac and 200 on my iPad and iPhone. .how do I fix

    Is this happening just from your Mac to your iPad and iPhone? In other words, what happens if you add a password on your iPad or iPhone instead? Does it appear twice on the Mac too?
