iOS data not Syncing to application on Mac (Dropbox)

I'm having some serious issues with sync on the Mac and iOS devices. I now seem to have split data. They are all using the same vault file on Dropbox but display different data. First noticed it when website passwords I knew I was updating did not work in another device. Right now I have some secure notes that are only on the phone, logins that only exist on the Mac and others only on the iPhone. In other words, a complete nightmare. Even disabling sync, clearing all the items, and re-enabling sync does not change it. Some items never get from one device to the other.

What is going on? I have double checked and both are pointing to the very same vaults on Dropbox.
