Enhancement Requests: Maintain history and detect the new password is from last 'X' PW

Now a days most of the sites ask to change PW very frequently.

1. Can for a login we maintain the complete history or audit trail which consist of the last (say 5) passwords, when changed (date and time) etc.
2. If 1 above is possible then can we detect the new password user is proposing for the site is not from the last 5 specified passwords?
