1password8 filling incorrect password

When I use the keystroke cmd-\ to autofill, a certain incorrect username and password are often being put into the fields and submitting automatically. Then I need to go back and do it again. The second time, cmd-\ usually fills the correct user/pass.
1. it's always the same incorrect user/pass that gets filled
2. it's only the first time after visiting a particular site's login page
3. but it can happen on the very next login to a different site

concerns:
1. it's wrong behavior
2. it's inconvenient
3. A user/pass combo is being submitted to the wrong site, potentially compromising the user/pass that is being submitted

It would be best to show you in a screenshare if you cannot replicate this.

This happens on Firefox and Chrome (haven't tested on Safari)

    Thanks for reporting this! We've actually sorted this out in version 8.9.4 of 1Password for Mac, and Login items won't be linked to Chrome inadvertently again. To resolve this issue, look for this specific Login that's always being filled, edit it, and remove the Linked App for Google Chrome and Firefox.

    Let me know how you get on with that.
