multiple password choices

Sometimes when I request a password for a given URL, it will suggest the correct one but also all the ones used previously. How can I archive, delete, purge, or whatever those bad choices. I only want the correct one to show.

I put this under Mac, but it also happens with IOS and iPad programs. I presently use Safari (latest) on all machines.

    Would you be able to show me a screenshot of what's happening here? Make sure you block out any sensitive information including usenames, email addresses, passwords, credit card numbers, your Secret Key, or 1Password account password.
