1Password 8 Chrome Extension is not retaining field values for saving logins

I'm trying to save a login for the Japan Postal Bank at the following URL:

https://direct.jp-bank.japanpost.jp/tp1web/U010101WAK.do?

There are three separate fields for the User ID. I properly fill them all in then right-click the 1Password extension icon and choose "Save Login" but the username field always remains blank as shown.

I tried with the same results in Firefox as well.

I have another login saved for the same site that I created a few years ago in an earlier version of 1Password (and don't remember how I did it). Strangely, when editing that login I only see (and can only edit) the final field of the three User ID fields in the username field, although they all populate properly when using the extension.

I'd love any advice for how to get this working.
