2FA not correct on other devices

So for some websites the 2fa doesn't work and is the wrong code and I have to use my phone. So for example I try to login to GitHub and the browser extension puts the wrong one-time password making me have to pull out my phone and type it in manually, and when looking at the codes on both devices they don't match up at all.
