Since you've already found an answer to your second question I'll reply to the first:
When do I enter the Secret Key? Is it the first time I install the Extension in a new browser or run the app on a new machine? What about on phones? Will I ever have to re-enter the Key? (just need to know how accessible the key needs to be)
You'll need the Secret Key the first time that you add your 1Password account to a new device or browser. Once your account is added to a device/browser you'll just need your account password (or biometrics) to unlock 1Password going forward.
I recommend that you download and save your Emergency Kit and keep it in a safe place.
