How to save custom forms?

I live in Canada, and Cortico is one of the largest tools for health care appointments.
I would love to be able to save the data from my whole family to fill that form.

The Health Care Number is type "number", so it sees as a credit card...
If I save a field with the name "date" it will fill the year, but I can't work out the month and the day.

    Thank you for bringing this to our attention, it seems 1Password is having a hard time detecting the fields on this page. I have filed an issue with our development team to see if we can improve the behaviour on this page in a future update.
