Saving Image lost all my info on a Login Item

Could someone please help. Here are the steps:

  1. I had an existing Login Item
  2. I edited this and added a One Time Password
  3. I saved the edits since you can't see the code without doing so
  4. I edited this login item again and added an image as a file
  5. I saved the edit
  6. After a few minutes of doing something else, I went back to the Login Item and both my One Time Password and Image are gone now.

I took the photo using the 1Password Take Photo, so it is not in my gallery. Is there a way to recover the photo somehow in my cache on my iPhone? or does anyone know if this is a glitch?

I tried a different Login Item with the above and it worked, so i have no clue why the first one didn't work. It had to have saved the One Time Password because you cannot enter the 6 digit code without saving.

Any insight is appreciated. Thanks.
