What am I missing: I can't pick a new icon or delete certain fields in a template...?
I'm trying to get a little more granular in how I use 1Password and not just make everything a login or a secure note. I am, however, quite perplexed by two things. The scenario is what I want to create vaccination records for myself and my family. I seemingly have two choices:
1) Use the medical records template, except it has a bunch of fields up at the top that I can't delete or re-order to be lower. This makes editing a bit more of a pain (I prefer to only see fields that are in use on that card).
2) Use a secure note, but when I click on the icon I don't get to pick from the existing 1Password icons, I have to go find my own icon. This seems like a needless hassle — I want to use the same medical icon as the Medical section uses. And I want to keep the notes in the medical section...but then I'm back to #1.
Am I missing something?

Thank you for reaching out! I use the Medical Record item to store my vaccination information. The Medical Record item is quite versatile and you only need to fill in the fields that apply to the type of information that you'd like to store. Only filled in fields will appear visible when you save the item.
You can then use custom fields for additional information. Here's an example:

Hi Dave. Thanks for the response, but see #1 in my post above. I was hoping I was missing something because not giving the customer control over the fields they see when they edit seems strange to me, but I guess that’s the way it is for now. It would be a welcomed improvement in the future to be able to delete all fields in my own cards.

Thanks for the reply. At the moment, 1Password doesn't support custom templates and instead comes with a variety of preset item templates. I can see how a custom template feature, perhaps created by modifying an existing category, would be useful and I've filed a feature request on your behalf.
For the time being, using a Secure Note as you mentioned in point number 2 is likely the best option. Secure Notes are very flexible and you can add custom fields to organize the information that you add. Here's a blog post that explains some of the possibilities:
Once you've created a Secure Note with the custom fields that you'd like, you can add a custom icon to the item and the save it as "Vaccine Template". Then, when you need to add a new item using this template, you can right-click on it and select Duplicate.
I know that this isn't the most elegant suggestion but I hope that it helps. 🙂
