How do items occasionally wind up in my Private Vault

I've reported this many times;
I have a Shared Vault and a Private Vault. The default for both my wife and myself for saving logins, etc. is the Shared Vault. That being said, occasionally I find items in the Private Vault.
How does that happen?

    I'd doublecheck that items are saved in the desired location by checking 1Password in the browser and 1Password ensuring the correct save location is set. A lot of folks think the setting will carry over from one place to the other.
