One-time password not working in Microsoft Tenants

Hello, a few weeks ago I noticed a strange and intermittent behavior when using a one-time pass code, it says "You did not enter the expected verification code. Please try again."

When displaying more details, I get this:

Error Code: 500121
Request Id: 9bba06a9-dc41-4ffb-8f97-73a5e5342800
Correlation Id: 56846d78-5076-46b2-8751-6dda2941996b
Timestamp: 2024-09-12T12:16:35Z

So, if I try again in several minutes it may work, but in some cases it doesn't. It's something intermittent, as if the one-time pass was passing the wrong code, even though it was valid and within the time limit.

I tried in Chrome, Edge and Firefox browsers and the problem persists. I also tried to return to the classic version of 1pass, the problem persists.

Does anyone know what's going on? Thank you!

    The first thing to check is whether the time on all your devices is accurate. Two-factor authentication is very time-sensitive, with one-time passwords having a validity period of 30 seconds. Any difference in the time of day on a device could cause the behaviour you're seeing.

    • What steps are you taking when using 1Password to autofill?
    • What steps were taken when it does work?
    • Are you referring to 1Password.com when you mentioned the classic version of 1Password?
    • Is the two-factor authentication code from your Login item different on 1Password.com compared to the 1Password browser extension?

    Time.is reports that my time is correct, except for a small difference of +0.018 seconds, which I believe has no impact.

    I asked 2 colleagues on my team to test it on their computers and the problem is the same. We tested it on all major browsers (Chorme, Edge, Firefox, Opera), without success. The desktop version was also tested on 2 different computers, same problem. I tested the Android app, same problem.

    This started a few weeks ago, and no changes were made. Sometimes it works perfectly, but most of the time the code is currently invalid.

    I ran a test on "Bitwarden" today, I exported my entire 1pass database and imported it into it. In some tests, the MFA code worked.

    Is there any configuration in the 1pass admin area that could be checked, some detail or the Timezone part?

    In response to your questions:

    What steps are you taking when using 1Password to autofill? We do not use auto-fill, we use copy and paste only.

    What steps were taken when it does work? It works intermittently. Usually clicking F5 several times, but it doesn't always solve the problem.

    Are you referring to 1Password.com when you mentioned the classic version of 1Password? Both the classic desktop version downloaded from the official website, and the web version used in the browser, my company's address https://mundo365.1password.com/

    Is the two-factor authentication code from your Login item different on 1Password.com compared to the 1Password browser extension? It's the same, but I don't use the extension anymore, it was something that wasn't working properly with the search.

    Thank you for your reply. There is nothing in the admin area to check. You mentioned:

    To confirm, does the MFA code from Bitwarden intermittently work as well? Does the same problem occur if you add the 2FA to an authenticator app like Authy or Microsoft Authenticator?
