Opening Credit Cards

I periodically receive the following message when opening a credit site in 1PW 4. Anyone else receive it and anyone know a solution. I uninstalled and reinstalled the Safari Extension and the Credit Card Site. Appreciate any help./Users/macpro/Desktop/Screen Shot 2013-11-02 at 2.17.31 PM.png
