More than one "username" field

This is what my media temple log-in looks like:

As you can see it has two "username" fields. ("Primary Domain" & "Primary Email")

1PW incorrectly fills both "username" fields with the same value.

Is it possible to label a field exactly so that the label in my 1PW called "Primary Domain" matches the form field "Primary Domain" and this gets filled in correctly???

Or does 1PW only work with 1 username field???

For step 2, be sure to enter all three fields that you 1Password to fill.
