Hey Terry, I'm not sure if you're trying to use version 3 or 4 of 1Password, so:

If you're using 1Password 4:

You need to install the version 4 browser extension, and remove the old one. Here are instructions:

Install 1Password 4 browser extensions

Uninstall 1Password 3 browser extensions

If you're using 1Password 3:

Please try following the linked instructions:

"Problem with database" in v3 extension in Safari on OS X Mavericks

(Give this a try even if you aren't using Mavericks.)
