How to create a login that uses more than UN and pwd?

On https://www.sofinco.fr/login.htm there are 5 fields required. The first ones are un and pwd, but then DoB is required as well.
I tried creating a section with the appropriate fields name I got from the source code, but these are not used.

    Please try creating a new login for that site using these instructions:

    Tried that, and only the un/pwd are saved.
    I checked, and all 5 fields are in the same form.

    When you select Show web form details for your Login item which fields do you see there?

    Hi sjk,

    Yes, it is now working. I had to manually save it and not rely on the auto save. And you are right, these additional fields do show up in the 1Password app.

    Thanks for letting me know you've got a functional Login item for that site now. :)

    We are working on autosave improvements to reduce the amount of manual saving as a workaround.
