How to add "Notes" while inputting a new login?

Hey guys,

When I try to edit a login information, I see the "Notes" field being displayed, however when adding a new login there is no "Notes" field, only "Username", "Password" and "URL". Am I missing something or it was forgotten?

    By the way, isn't the "Login" icon wrong when adding one? Shouldn't it be the lock icon?

    The icon on the add login item screen definitely looks wrong. I believe that the notes field should be present on the add login item screen as well. Good catches in both cases! I will bring this up with our development team.
