Field duplication when trying to add a new login

With the newest version (4.0.0b8 build 20140115) the inputs are being duplicated:

Plus, just to reinforce that the icon is wrong - that's not the correct "Login" icon

    I'm not able to post any screenshot as an image when replying to a topic. Not sure why, as I often copy the image URL and not the link that Dropbox generates me. Guess the markdown interpreter is not working well?

    I'm also sending it to the email address :)

    I passed on your screenshot to our developers and it looks like a bug in 1Password. We will fix this in an upcoming update, thanks for reporting it!
