1Password keeps saving the same logins

On both my mac and pc, 1password keeps asking to save login info even though it hasn't changed. I either have to say not now each time or save and I can end up with an unlimited number of the same logins. How do I tell it to stop asking? It is becoming more of a nuisance than a help

    What versions of 1Password, browser, and browser extension are you using on each platform (Mac and Windows)?
