Hi @zharry,
Let's try restoring your database from the latest backup:
- Open 1Password.
- Go to File > Restore… in the menu bar.
- In the resulting window, choose the most recent backup (it will show you your item counts for reference).
- Click Restore.
Note: If you have created any items since your last backup, please select any new items and choose File > Export > Selected Items... to export them to a 1PIF file. Once you've restored from the backup, import the 1PIF file back into 1Password by choosing File > Import... in the menu bar. Please note that the 1PIF file in not encrypted, so be sure to securely delete it once it's successfully been imported (trash the 1PIF file and select Finder > Secure Empty Trash...).

A 1PIF file is one that's using that file format, defined in the 1Password 4 glossary:
1Password Interchange Format. This is an unencrypted file format that 1Password can use for quick data migration projects. Because it's unencrypted, please securely delete it when you're done with it.
The File > Export > … commands can export to that format:
All backups have same error message.
You're getting a "database image is malformed" error when trying to create an item in every restored backup that you have?
