Login window with three entrys

I need three different entries:

domain
username
password

1 Password always enters my username in domain AND username and I have not found any way to teach it that this user has three different fields to fill out. A similar problem exists in my online banking, where I also have 2 "users" I have to fill in besides the password.

How can this be done?

Does it help at all if you re-create the login and save it manually? That does sometimes resolve problems like this.
