Browser extension doesn't work for Apple/iCloud

I'm using 1Password 4 for Mac, not the MAS version. I have the most recent version of the Safari extension installed. 1Password works fine except at apple.com and iCloud.com. The login is there in 1Password if I open the full app, but it doesn't show up in the browser extension or in 1Password mini. Any ideas? I've tried recreating and renaming the login to no success.

    Just edit the relevant item and check the display field, towards the bottom, is set to Always.
