1password4 won't fill user name

When I login to my bank acct (https://banking.patelco.org/Account/LogOn), 1p fills in password, by won't fill username. All other accounts work .

Have you tried re-creating and saving the login manually?

Have you been able to get a working Login item for the Patelco Credit Union site using @Stephen_C's suggestion for creating a new one? Also, which web browser and version of the 1Password extension are you using? Thanks!
