Safari extension icon not appearing [resolved]

I have the 1Password Safari extension installed, but the key icon is not appearing. I have closed and reopened Safari. I have rebooted my computer. Still no icon.

I am running Mavericks 10.9.5 and the latest Safari upgrade.
