Safari Extension Hanging after Login

I'm running the latest version of iOS (8.1 at this writing) with 1Password 5 and the Safari extension enabled. I am unable to get the extension past the password screen. On a Safari page, I open the sharing control, click 1Password, then enter the password. The input form checks the password, says 'Unlocked' and then I get a bare looking screen with just a Cancel button. I have tried restarting the iPad and removing/reinstalling 1Password to no avail. For what it's worth, the 1Password app is working just fine.

    where you able to solve this by adding a URL to your login item?

    Problem Summary

    I am not sure if I am having the same problem, but Safari now hangs on me. Per the console logs, the 1Password extension is having problems, and can lock the whole laptop which forces a reboot. Due to this issue I have deactivated the 1Password extension in Safari, and migrated to Chrome, but it would be helpful to find a resolution.
