logins / passwords NOT updating, just adding

i find when i have changed a password for a login, instead of updating it, it will create a new entry, then i cant keep track of which is the correct one. this really needs fixing.

That should not happen if you follow the correct procedure for changing a saved password. Are you doing that when you have that problem?
