Bug report: 1Password Mini 5.0.2 Mac filling in my first name for both Firstname and Lastname fields

1Password made a mistake - filling in "Jeffery" in both fields, first name and last name. Ergonis software support told me "Could you please send a bug report to AgileBits. We’ve had that problem several times recently, and it’s not only cumbersome for us, but also risky for you!"

I had this problem filling in more than one site using 1Password Mini on Yosemite 10.10.1 and iPassword 5.0.2.
