login different than checkout

Given it's happening on a checkout page that makes it tricky for us to test unless it's a common site.
Open 1Password and find your Login item. Right click on the item in the Item List part of the window and select the Duplicate option. This will create a copy (just in case).
Go the the checkout page where you're having the problems and paste your password in as you've had to do before but don't click the continue button. Instead click on the icon for the 1Password browser extension and select the Save new Login option (see below)
We don't want to save a new Login though, we want to get your existing Login working on this second page. So you want to change the option from Save new Login to Update existing Login (see below). NB. This will only be possible if you have an existing Login item that matches based on the URL.
The textbox where you could type a name now becomes a menu where you can select which Login item to update, assuming you have more than one for a single site (see below)
So what I'm trying to do here is to see if we can update the web form details for this Login item so that it correctly identifies this page and fills properly.
If that doesn't work or even worse, the Login item is mangled as a result then you know you have a copy from the first step. You can delete the knackered one and rename the copy to return to your previous state.

It turns out I've given you bad advice and that set of steps isn't recording the extra fields I hoped it would so I do apologise.
Instead, what might be worth a shot is creating a basic Login item from within the main 1Password window. Create a new Login item and copy your username, password and the website URL across. This 'clean' Login item might work for both the main login page and this confirmation login page. If that doesn't have any impact please do let us know.
